For general questions and requests for prescription refills, send a fax, call between 8:30 and 4:30 M-F, or contact us through our patient portal.

  • Please allow 48 hour notice for all refill requests via fax, phone or patient portal.
  • For questions about billing and payments call Landmark Billing Systems, Inc. (815-965-8505), our billing services company M-F, 9AM – 4:30PM. Their friendly and competent staff is available to help you with any billing concerns you may have.
